According to the Society for Vascular Surgery 35-40 Million Americans suffer from Chronic Venous Insufficiency (CVI). The U.S. Department of Health and Human Services reports that about 50 to 55 percent of women and 40 to 45 percent of men in the United States suffer from some type of vein problem. What are Varicose Veins?

Varicose veins are unsightly twisted red and blue lines that commonly appear on the legs. Varicose veins are caused by blood pooling in the veins that carry blood back up to the heart, and may appear during pregnancy or as you age. Healthy leg veins have valves that keep blood flowing to the heart. Chronic Venous Insufficiency develops when the valves stop working properly and allow blood to flow backward (i.e. reflux) and pool in the lower leg veins.

While any vein in the body can become varicose, the legs are the most common location due to the pressure exerted on those veins through standing and walking.

For some people with varicose veins, the ugly blue leg veins usually pose little more than a cosmetic concern. However, large varicose veins may also begin to cause pain, restlessness and swelling in the lower legs and ankles. Some varicose vein sufferers may develop complications from their veins, including leg and foot ulcers or an increased risk of cardiovascular problems.

Endovenous Laser Therapy (EVLT): 

Endovenous ablation is a treatment that uses heat to close off the affected varicose vein. After a local anesthetic is injected into the leg, a laser fiber is inserted into the vein under ultrasound guidance and advanced to the end of the vein. Additional local anesthetic is then injected surrounding the area of the vein to be treated. Once the anesthetic is placed, the proper position of the laser demonstrated by ultrasound, the laser is turned on, pulled back through the vein, and removed. Following the procedure, a small Band-Aid is placed on the site, and a compression stocking is placed on your leg.

Because endovenous laser treatment is considered a minimally invasive procedure, risks associated with the procedure are relatively small.  Some uncommon risks might include:

Are the Results Permanent?

EVLT  is successful in permanently removing varicose veins nearly 100% of the time. However, other veins can become varicose at the same location, resulting in the need for subsequent procedures. Micro-Phlebectomy: 

The Micro Phlebectomy , also know as “Stab Phlebectomy” is often done at the same time as the EVLT.  Under local anesthesia, the diseased veins are delicately extracted using specially designed hooks. The incisions are truly stab wounds and are so small, no stitches are required and there is virtually no scarring.

How does the Procedure Work?

Stab Phlebectomy is typically done under light IV sedation or local anesthesia, so patients do not feel any discomfort during the procedure. The veins to be removed are carefully marked so the doctor knows precisely where to make the incisions for removal. Incisions used to remove the veins are so small, no stitches are required. Veins are removed in small sections until the entire vein is removed. The procedure is relatively quick, although the specific length of time will vary based on the number of veins the patient will have removed.

What is the Recovery Like?

After the procedure, the patient is fitted with compression stockings to reduce the risk of blood clots and swelling and promote healing. The stockings will need to be worn for about one week after the procedure. Most patients find that mild swelling and discomfort after the procedure can be effectively managed with over the counter pain medications. Symptoms usually abate after the first 24 hours. Patients are allowed to resume normal activities as early as the day after the procedure, although strenuous exercise is discouraged for approximately two weeks.

Sclerotherapy:

This procedure has been around for nearly a century and is still considered one of the most effective ways to treat spider and varicose veins. A foam saline solution is injected directly into the targeted veins, causing them to collapse and disappear from view. Treatments take between 15-30 minutes and patients can go right back to their normal activities.

Groin Dissection:

While Endovenous Laser Therapy (EVLT) for varicose veins has revolutionized treatment of Chronic Venous Insufficiency (CVI), a small percentage of varicose veins are too large and swollen to be closed by laser vein therapy. Leg Vein Surgeons solved this problem by surgically removing a segment of the malfunctioning vein  and close the other end with RF Ablation (laser treatment). This removes the vein from the system with no chance of re-opening.
